Services are designed to minimize the impact of illness, emotional distress and injury on studies and work. UHS provides on-campus medical and mental health care and coordinates supplemental needs for off-campus care through a network of community specialists and hospitals. The Clinical Services Program within the UHS includes urgent care, primary care and specialty clinics, laboratory, pharmacy, physical therapy, radiology, and a major medical insurance plan. I.CLINICAL CARE In accordance with Standardized Procedures and consistent with community standards and UHS policies and procedures, provides direct patient primary care. 1.Takes appropriate health history according to presenting complaint and physical findings including pertinent negatives. 2.Conducts an appropriate physical exam corresponding to presenting complaint, and history. 3.Formulates appropriate medical and nursing assessment of patient problems. 4.Formulates appropriate disposition and/or treatment plan. Includes: ordering and interpreting diagnostic test(s), implementing therapeutic treatment modalities (including medications), and providing appropriate patient education. 5.Integrates patient education and instruction in health maintainence and disease prevention into practice including guidance in nutrition, risk factors, episodic illness, contraception and self care, and behavioral health. 6.Counsels and provides emotional support to patients as appropriate. 7.Discriminates between normal and abnormal findings and determines when to collaborate or refer to a physician and/or specialist for further evaluation and/or treatment. 8.Evaluates the total health care needs of patients and develops plans for comprehensive care. 9.Works collaborately and cooperatively with the health care team in the management of medical, gynecological and urgent conditions. 10.Provides feedback and/or follow-up to patients in a timely fashion. 11.Determines the need for appropriate follow-up care and arranges for appropriate referrals of patients for further evaluation. 12.Consults colleagues when appropriate and in a professional manner.
II.DOCUMENTATION OF PRACTICE Maintains professional standards of documentation. 1.Records content of all patient encounters within the patient’s electronic medical record utilizing standardized templates (or progress note using SOAP) and workflow. 2.Medical documentation is easily interpreted by all members of the health care team. 3. Charts in a timely fashion. 4. Charting reflects the process of care, patient progress and current patient status. All communications with patient should be recorded on EMR. 5. Documents on-going and significant problems on the Problem List. 7. Updates current medications, allergies and significant medical history at each visit. 8. Completes appropriate forms including, but not limited to, work-related injury forms, encounter forms, ancillary forms and urgent care forms. 9.Appropriate referrals and documentation on EMR include in visit orders, referrals, request for insurance authorization, etc.

IV.LEADERSHIP/PROFESSIONAL GROWTH A.As a nursing professional, demonstrates an ability to communicate knowledge both formally and informally. 1.Collaborates with other disciplines in planning, developing, gathering, implementing, and evaluating patient education materials. 2.Participates in the orientation of new employees. 3.Acts as a preceptor providing instruction and guidance in a constructive manner. 4.Serves in a teaching capacity and as a resource person for the development of other health care workers, individual patients, volunteers, and groups. 5.Engages in self evaluation and goal-setting to maintain and/or improve quality of own performance. 6.Maintains RN, NP, NP Furnishing, DEA, and CPR (ACLS if appropriate) licensing and certification. Provides timely documentation to UHS. B.Contributes to the professional development of nursing and Nurse Practitioner practice. 1.Demonstrates and supports professionalism in nursing and Nurse Practitioner practice. 2.Develops and maintains area(s) of clinical concentration or specialty that are consistent with UHS needs. 3.Serves as a resource person for other nursing staff in the enhancement of basic nursing practice. 4.Takes responsibility for own educational needs and keeps current on literature pertinent to ongoing advanced practice. Shares with staff information from research in professional literature. 5.Attends and participates constructively in staff meetings. 6.Participates on UHS committees as assigned. 7.Participates in the development of standards, policies and procedures essential to the delivery of quality medical, nursing, and preventative health care services. 8.Assists in identifying NP staff educational needs and issues in NP practice. Prepares professional literature to share with other NPs and/or appropriate staff. 9.Contributes to (and participates) in Quality Assurance and Peer Review activities as needed. 10.Facilitates problem-solving. 11.Contributes constructive concerns and/or ideas for improving practice efficiency and work relationships.

B.Knowledgeable in emergency policies and procedures. 1.Knows emergency/disaster plan. 2.Knows use and location of emergency equipment at a level appropriate to the assigned unit. a. Initiates basic CPR as needed. b. Initiates ACLS as approprate to training and resources. C.Demonstrates understanding and consistently follows Standard Precautions. 1.Implements UHS Bloodborne Pathogen and all Infection Control policies. D.Adheres to hours of work and payroll responsibilities. 1.Demonstrates flexibility in work schedules which may include rotation to other clinical areas as needed and may include evening and weekend shift assignment.
VI.OTHER DUTIES AS ASSIGNED BY SUPERVISOR.•Must have completed a minimum of three years of practice as a Nurse Practitioner Hold and maintains: •Active State of California RN License and NP Certification •NP Furnishing Number and DEA License. •Active Board Certification. •BLS Certification.
