Incumbent is accountable for the management and coordination of all pre- and post-procedural patient care for the Department of Radiological Sciences, Section of Interventional Radiology. Duties include assessing health needs, planning and evaluating care for patients with complex health problems. Participating with outpatient clinical care and follow up with the IR team/residents. Responsibility for care and organization of procedures and follow-up, outpatient tracking, follow-up and referrals, overseeing the scheduling area and discharge patients (including discharge summary, instructions, and medications). Responsible for the reviewing critical labs, follow-up of results with patient and medical staff, conducting in-depth interviews with patients and/or family, constructing comprehensive medical histories, performing initial and periodic physical examinations, and exploring patients interpretation of health needs. Incumbent will identify and treat patients and manage treatment of hospitalized patients. Perform clinical assessments and care management services that include construction and interpretation of histories and physical exams.
